State Immunization Requirements
2011 - 2012

Rules for new students entering a Colorado school for the first time

New students must follow requirement for the grade to be entered as indicated below, AND:

  1. need 5 DTP/DTaP/DT/Td - type vaccines, but may enter with 4 doses if #4 was given at/after age 4. A student needs no "P" (Pertussis) if age 7 – 9 years old.
  2. need 4 polio doses, but may enter with 3 doses if #3 was given at/after age 4
  3. need hepatitis B series (3 doses) - required for all school grades.
  4. need 2 MMR (measles/mumps/rubella) shots for all school grades.
  5. need 2 Varicella/chickenpox shots - 2 doses at least 28 days apart (prefer 3 mo.) or disease history by health provider or lab test for kindergarten and grade 1, 2, 3 and 4.
  6. need 1 Varicella/chickenpox shot or disease history in grades 5, 6, 7, 8, 9 10 and 11 for the 2011-2012 school year. Grades 11 and 12 do not require Varicella for 2011-2012.

Colorado immunization requirements also exist for:
- children in infancy through 4+ years old who attend child care or preschools
- college students who attend state-supported schools that have any on-campus residence housing

NOTE: Medical, religious or personal exemptions to Colorado immunization requirements are possible for school and/or daycare attendance, and require a signature from a physician or parent on a Colorado Certificate of Immunization. EXEMPTIONS:
If immunization is against your religious beliefs, you must sign the "Religious Exemption" of the Colorado Department of Health Certificate of Immunization. If your child cannot be immunized for medical reasons, a physician must sign the "Medical Exemption". If you have a personal (philosophical) belief opposed to immunization, you must sign the "Personal Exemption".
